National guidelines for CPR

The recommendations cited below come from studies conducted by ILCOR.

The guidelines we present here are the result of a rigorous synthesis of decisions drawn from multiple scientific sources, originating from national publications devoted to Cardiopulmonary Resuscitation (CPR) and Emergency Cardiovascular Care (ECC). These guidelines, which serve as the basis for training in clinical and pharmacological practice in Canada, result from the aggregation of the scientific data collected and the recommendations arising from the most recent evidence evaluations, developed by the International Liaison Committee on Resuscitation (ILCOR).

Summary of the guideline development process

The development of CPR and ECC guidelines relies on meticulous efforts to analyze relevant scientific data by a set of key stakeholders. Established by ILCOR, this process makes it possible to provide clear recommendations on how to deliver care in cardiovascular emergency situations.

These guidelines for CPR and ECC are based on a body of evidence and scientific research on an international scale, constantly evaluated by ILCOR. The ILCOR evaluation process mobilizes hundreds of scientists and international resuscitation experts, who review, discuss, and debate thousands of peer-reviewed publications. As part of this process, the ILCOR working groups define the priorities of the topics to be reviewed, in close collaboration with resuscitation specialists. Once a topic is approved for evaluation, a systematic review is carried out, led by a specialized knowledge synthesis unit or a systematic reviewer, in collaboration with ILCOR content experts. At the end of this systematic review, the ILCOR working groups analyze the evidence and draft a preliminary document of the Consensus on Science with Treatment Recommendations (CoSTR) statements, which is then made available to the public to gather comments. The drafting groups of the ILCOR working group then compose a summary of the CoSTR taking into account the comments received. This summary is then used by resuscitation councils, such as the American Heart Association (AHA), to compose their own guidelines while taking into account their resources, the realities of training, and the specific features of their health care system (AHA and Heart & Stroke, 2019).

The keys to the guideline development process

ILCOR is a committee created in 1992 with the goal of serving as a liaison forum among the leading resuscitation organizations worldwide. Its mission is to develop, with the help of its members, guidelines on resuscitation. It plays a crucial role in the evaluation, discussion, and debate of the thousands of peer-reviewed publications (ILCOR, n.d.).

To issue its recommendations, ILCOR forms working groups made up of hundreds of researchers and experts who are tasked with conducting systematic reviews on specific topics. During the development of the guidelines, the ILCOR working groups evaluate data from thousands of scientific works and address the issues to be considered. They select the priority themes to be evaluated, with input from resuscitation professionals (AHA and Heart & Stroke, 2019).

In order to enable ongoing evaluation of the evidence, ILCOR calls on experts in systematic evaluation. They evaluate the existing evidence relating to the priority themes selected by the ILCOR working groups and carry out meta-analyses and syntheses of the scientific data. The data gathered from the systematic evaluation of each priority theme is then analyzed by the working groups and compiled into CoSTR documents. The summary of the CoSTR documents is then used by resuscitation councils, such as the AHA, to develop their guidelines.

The guidelines that follow are based on the official recommendations from ILCOR and the resuscitation councils that are its members. They represent the foundations of our training programs aimed at preparing health care professionals, as well as the Canadian population, to manage emergency situations.

Our guidelines are therefore developed based on research and data from several international scientific works. These works contain information from medical research carried out on an international scale and serve as the basis for the development of the CPR and ECC recommendations.
